A. HIGH SCHOOL DEPARTMENT
• Original copy of Form – 138 signed by the principal of the former school
• Certification of good behavior during his/her stay in school last attended signed by the principal or guidance counselor
• General average not lower than 80% and with no failing grade in any subject
• Written conformity with school rules and regulations attested by the parent or guardian
• Photocopy of birth certificate issued by the NSO
• Passing the qualifying examination
• Interview with the principal or the guidance counselor
B. COLLEGE DEPARTMENT
• Honorable Dismissal and Transcript of Records from the previous school attended
• Certificate of good moral character
• Freshmen transferees must pass the entrance examination
• Two (2) copies of latest 1½ x 1½ photo
• Photocopy of birth certificate issued by the NSO
• Interview with the Dean
