ADMISSION REQUIREMENTS
A. FOR INCOMING FIRST YEAR STUDENTS
- Original copy of Form -138 signed by the principal
- Passing the entrance/classification test
- Photocopy of birth certificate issued by the NSO
B. FOR OLD STUDENTS
- Form – 138 signed by the principal
- Registration form filled out and duly signed by the student, parent and enrolling teacher
- Summer grade for students who attended summer classes
- Interview with the guidance counselor upon the recommendation of the former adviser and teachers
C. FOR TRANSFEREES
- Original copy of Form – 138 signed by the principal of the former school
- Certification of good behavior during his/her stay in school last attended signed by the principal or guidance counselor
- General average not lower than 80% and with no failing grade in any subject
- Written conformity with school rules and regulations attested by the parent or guardian
- Photocopy of birth certificate issued by the NSO
- Passing the qualifying examination
- Interview with the principal or the guidance counselor
A.   FOR INCOMING FIRST YEAR STUDENTS
- Present Form – 138 to the cashier and pay the entrance/classification examination fee.
- Take the entrance/classification test on schedule specified on the permit.
- Come back on the designated date when the results will be released. The list of successful examinees will be posted at the entrance of the campus. Take note of your number in the list.
- If you qualify, go to Rm. 105 and present your number to the enrolling teacher for confirmation.
- Fill up the enrollment form completely and accurately.
- Present the enrollment requirements to the cashier and pay the miscellaneous and tuition fees.
B.   FOR OLD STUDENTS
- Go to Rm. 105 and ask for an enrolment form.
- Fill up the enrolment form completely and accurately.
- Present the enrolment requirements to the cashier and pay the miscellaneous and tuition fees.
C.   FOR TRANSFEREES
- Present Form – 138 duly signed by the principal and certificate of good character to the GJC principal. Interview of the applicant with either of the parents will also be done on this schedule.
- Secure schedule for the qualifying test after payment of the examination fee.
- Take the qualifying test and come back on the date when the result will be released. Inquire at the Guidance Office.
- Proceed to Rm. 105 and present the result of the test to the enrolling teacher. Fill up the enrolment form accurately and completely.
- Present the enrolment requirements to the cashier and pay the miscellaneous and tuition fees.
A.   FOR FRESHMEN
- Original copy of Form -138 ( Report Card)
- Certificate of good moral character
- Passing the entrance examination
- Two (2) copies of latest 1½ x 1½ photo
- Photocopy of birth certificate issued by the NSO
- Interview with the Dean
B.   FOR TRANSFEREES
- Honorable Dismissal and Transcript of Records from the previous school attended
- Certificate of good moral character
- Freshmen transferees must pass the entrance examination
- Two (2) copies of latest 1½ x 1½ photo
- Photocopy of birth certificate issued by the NSO
- Interview with the Dean
- Get an Assessment Form from the Dean’s Office. Fill it up.
- Present the Form to the Dean/Registrar to assess the number of units.
- Proceed to the Accounting Office for the assessment of fees. Pay the amount indicated in the Assessment Form.
- Get the Registration Form from WINDOW B of the Registrar’s Office.
- Fill up the Form completely and accurately.
- RETURN the Form to Window B and GETÂ your student copy.
Note:
1. Education students must have a general average of at least 85% in their secondary course.
2. BSA students must have a general average of 83% - 85% in their secondary course. If the gen. average is lower than 83%, the student’s grades in Math & English must be 83 & above in order to be admitted in the Department or his entrance examination is 85% & above.
If the student is a GJC high school graduate, only 80% gen. average is required.
